Failure Investigations

Pipeline operators need to know the root causes of pipeline failures to avoid recurrences and to reduce risks to pipeline safety. If you send ruptured or leaking pipes or components to Kiefner and Associates, Inc., our experts will assess the proximate causes of failure. We also consider any operational and environmental factors that you can supply to determine the root cause of failure.

Our usual turn-around time for a written report on the cause of failure is thirty days or less. If your company is involved in a failure that is investigated by the National Transportation Safety Board and/or Pipeline and Hazardous Material Safety Administration (PHMSA), we can help by serving as a party to the investigation.
